Option 1: Patients Self-Schedule Online

• Give your patients a copy of the requisition form 
• They can call and make their appointment 
• Their lab results are sent to you from the laboratory you choose

Option 2: Your office orders for the patient

• Your Medical Assistant sends us the patient lab order

• Call to schedule a appointment or have the patient give us a call

Your patient’s lab results will be sent to you from the resulting lab. Not Walls Phlebotomy.

We are direct pay from the patient.

We will also send the patient a super bill for reimbursement from their insurance company.
