To schedule an appointment please follow these steps:

This helps us match your info with the lab orders. Please be aware that lab orders do not have your contact information on it, we need this information to contact you.

 Step 2: Send Your Lab Order

Have your provider fax your lab order, or email us a copy. We will match this information with the information above.

Step 3: Reserve Your Appointment

$100 Mobile Collection Fee

Designed for patients who only need occasional laboratory collections without a membership.

To reserve your appointment, payment is required in advance. Once payment is received, we will contact you to confirm your appointment date and time.

 

Wellness Plan

$10/month + $50 draw fee per visit

Save 50% on mobile phlebotomy visits. Ideal for patients needing regular lab collections.

Our Membership Plan is ideal for seniors and patients who require routine blood draws on a weekly, monthly, or ongoing basis. By joining, you receive a reduced visit rate while helping support our nonprofit mission.

Membership fees help us expand services, purchase supplies, and increase access to convenient in-home phlebotomy for patients throughout the community.

Limited memberships available due to the size of our nonprofit organization.

Step 4: We Come to You

A certified phlebotomist will visit your home, office, or care facility, collect your specimen, and deliver it to your selected laboratory.

Membership Plan

  • If no specimen is collected and no supplies are used, the $50 draw fee will be refunded.

  • The $10 membership fee is non-refundable.

One-Time Visit

  • If no specimen is collected and no supplies are used, a $50 refund will be issued.

  • Travel and scheduling fees are non-refundable.
