Membership Plan

$10/month membership + $50 draw fee per visit

Our Membership Plan is ideal for seniors and patients who require routine blood draws on a weekly, monthly, or ongoing basis. By joining, you receive a reduced visit rate while helping support our nonprofit mission.

Membership fees help us expand services, purchase supplies, and increase access to convenient in-home phlebotomy for patients throughout the community.

One-Time Visit

$100 mobile collection fee

Designed for patients who only need occasional laboratory collections without a monthly membership.

Your appointment will be scheduled after payment is received.


Step 4: We Come to You

A certified phlebotomist will visit your home, office, or care facility, collect your specimen, and deliver it to your selected laboratory.

Refund Policy

Membership Plan

  • If no specimen is collected and no supplies are used, the $50 draw fee will be refunded.

  • The $10 membership fee is non-refundable.

One-Time Visit

  • If no specimen is collected and no supplies are used, a $50 refund will be issued.

  • Travel and scheduling fees are non-refundable.

Important

  • Once collection supplies are used, fees are non-refundable.
